For your convenience, please print and fill out the forms below before your first session.

*Please bring your insurance card, picture ID, and these forms to your first appointment, and be prepared to pay your insurance copay or deductible.

*Prior to your first appointment, it would be very helpful for you to contact your insurance company. Be sure to learn if you have a deductible (and if it has been met for the year), copay for counseling sessions, and if your insurance requires pre-authorizations or has a limit for total number of appointments. Keep in mind that you are empowered to learn this information and are responsible for any charges not covered by your plan.
