Your First Visit

For your convenience, please print and fill out the forms below before your first session.
Please bring your insurance card, picture ID, and these forms to your first appointment, and be prepared to pay your insurance copay or deductible.
Prior to your first appointment, it would be very helpful for you to contact your insurance company. Be sure to learn if you have a deductible (and if it has been met for the year), copay for counseling sessions, and if your insurance requires pre-authorizations or has a limit for total number of appointments. Keep in mind that you are empowered to learn this information and are responsible for any charges not covered by your plan.
